Registration Paperwork

* Please note that even if you register online through Willows Little League, you will still need to complete a Medical Release Form as well as provide proof of birth date if a copy is not already on file.  Your coach or a member of the league will notify you if additional information is required.
  
In addition, if you are volunteering for the first time a copy of your driver license or ID will also be required.  You do have an option to upload this file when registering.
